Technical Assistance:
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 04/19/2021 and concluded on 04/29/2021. The in-charge person was contacted by telephone to initiate the inspection. There were 12 children present and 3 staff. The inspector emailed the in-charge person a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
Based on a record review the center did obtain documentation that each child had a physical examination by or under the direction of a physician. Evidence: Child #2 (enrolled 01/19/21) and child #4 (enrolled 01/17/21) did not have documentation of having physical examination under the direction of a physician.

The following corrective actions were provided during the exit interview: parent has been contacted and is working on obtaining documents and signatures

Description:
Based on a record review the center did obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: Child #2 (enrolled 01/19/21) did not have an immunization record signed by the physician.

The following corrective actions were provided during the exit interview: parent has been contacted and is working on obtaining signatures
