An unannounced focused monitoring inspection to ensure correction and compliance per the Intensive Plan of Correction issued on 04/07/2024 was conducted on 05/30/2024 from 10:00 a.m. to 10:25 a.m.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Repeat Violation Systemic Violation Based on observation, the center failed to ensure that where playground equipment is provided, resilient surfacing and the fall zones comply with the minimum safety standards. Evidence: 1. The pea gravel around the blue climber measured two inches on the left side (opposite of the red climber), five inches on the right side, four and five inches behind the ladder and four and a half inches beside the slide. 2. The pea gravel around the red climber measured five and a half inches in the back and five inches to the right (toward the fence) 3. The pea gravel did not meet the minimum requirement of six inches. 4. The red climber requires a six-foot fall zone; however, the fire truck climber was 57 inches from the red climber and within the fall zone.
