Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was conducted on October 19, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were no children present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two caregiver records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review, provider did not ensure 1 of 2 children have documentation of a physical examination within 30 days of the first day of attendance.
Evidence: The record for child #2 (start date June 28, 2021) did not have a physical.
Plan of Correction:
Parent has requested for the physician's office to fax the physical directly to the provider.
