Comments:
An unannounced renewal inspection was conducted on December 12, 2023 from approximately 1:10 pm to 2:30 pm. Provider and two assistants were present with eight children ranging in age from infants through preschool age. Children were observed napping. Three caregiver and five child records were reviewed. Violations found are noted on the violation notice.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on review, provider did not ensure all caregivers have a completed central registry search within 30 days of beginning employment.

Evidence: The record for caregiver #3 (start date September 2023) did not have a completed central registry search. The provider had documentation of a portability request however that did not apply to this individual.

Plan of Correction: The Central Registry Search will be completed and submitted through the online portal today.

Standard #: 8VAC20-800-500-C
Description: Based on observation, provider did not ensure stationary outdoor playground equipment is placed at least 6' from the perimeter of other play structures of obstacles.

Evidence: A two slide swingset was installed in the backyard for the children to use. The rock climbing wall portion is approximately 16" from the privacy fence. The wood border of the play area is approximately 3' from the foot of the slides.

Plan of Correction: The children will not use the stationary playground equipment until the structure can be moved to allow 6' of free space around the exterior.
