Comments:
An unannounced monitoring inspection was conducted on May 6, 2024 from approximately 1:50 pm to 3:00 pm. Provider and assistant were present supervising seven children. Children were observed napping and waking up preparing for pick up. Two caregiver and three child records were reviewed. Violations found are noted on the violation notice.
Based on review, provider did not ensure shelter-in-place procedures are practiced a minimum of twice per year.
Evidence: There was not documentation of shelter-in-place drills completed in 2023.
Plan of Correction:
One shelter-in-place drill has already been conducted in 2024. Provider will make sure at least two drills are conducted this year and documented.

Description:
Based on review, provider did not ensure 1 of 3 child records has documentation of required information.
Evidence: The record for child #2 had documentation of a food allergy that was signed by a physician. There was not documentation of actions to take in an emergency situation.
Plan of Correction:
Provider has already requested a copy of a allergy action plan. The parent stated the child has an appointment with the allergist and she will have the form completed. Provider stated she does not serve this food to any of the children.
