Comments:
An unannounced monitoring inspection was conducted on 1/8/2024 from 3:20 pm to 3:50 pm. There were 12 children in care, ages six years to ten years, in one group under the care of two staff. The Licensing Inspector reviewed three children?s records, two staff records, and other program records. The children were observed in free play indoors and snack.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the center failed to obtain some required information for a child?s record.
EVIDENCE: The record for Child 1 is missing the names, addresses, and phone numbers of two designated people to call in an emergency if a parent cannot be reached. Only the parents are listed.
Plan of Correction:
The information will be obtained from the parents and the record will be updated.
