Comments:
A monitoring inspection was conducted on October 11, 2022. There was one household member child present with caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. A total of one child record, one caregiver record and two household member records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review, provider did not ensure 1 of 1 child records has documentation of required information.
Evidence: The record for child #1 (start date 7/27/2022) did not have a physical examination before within 30 days after enrollment.
Plan of Correction:
Provider will get a copy of the child's most recent physical and place it in the child's record.
