Comments:
A monitoring inspection was conducted on November 29, 2022. There were three children present with one caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. A total of three children?s records and one caregiver record were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observation and review, provider did not ensure the criteria on the health and safety checklist is met at the time of the monitoring visit.
Evidence: The following was found during inspection: 1. Child #1 (start date 1/3/2021) did not have a immunization record. 2. Child #1 (start date 1/3/2021), child #2 (start date 2/28/2022) and child #3 (start date 6/28/2021) did not have a physical examination results within 30 days after enrollment. 3. The bathroom used by children did not have toilet paper.
