Comments:
An unannounced monitoring inspection was conducted today. There were 10 children present with 2 staff. The physical plant, staff records, children's records, evacuation logs, attendance logs, emergency supplies, medications and authorizations and menu's were reviewed. Based on a review of children's records, the center failed to ensure that all required documents are included in each record of each child enrolled.
Evidence: Child #2's record does not include documentation of work phone numbers for parents.
Plan of Correction:
I will contact the parents for the required information.
