Comments:
An unannounced monitoring inspection was conducted on 4/2/2024 from 12pm to 1:10pm. There were 42 (infants to preschool) directly supervised by 10 staff. The physical plant, 3 staff records, 5 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Description: Based on review, the facility did not ensure that staff records were maintained with the required minimum information.
Evidence: reviewed 3 new staff records and found that Staff 1 was missing documentation of 2 reference checks.

Plan of Correction: Reference checks will be placed in file today.

Standard #: 8VAC20-780-240-C
Description: Based on review, the facility did not ensure that orientation training was appropriate to the age of the children in care and include all of the required topics.
Evidence: reviewed the orientation training forms and found that they did not review the following items: prevention of shaken baby syndrome and abusive head trauma, including procedures to cope with crying babies or distraught children;

Plan of Correction: We will update the teachers' orientation training form to the most recent one before 4/30/2024.
