Comments:
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A non-mandated monitoring inspection was conducted on October 20, 2020.The director was contacted by telephone to initiate the inspection. There were 6 children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed two children records, and five staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility.
The licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Based on review of staff files, the center failed to ensure that a sworn disclosure statement had been completed prior to the first day of employment.
Evidence: 1. Staff #1's sworn disclosure statement was dated 09/02/2020. Start date for Staff #1 was 08/25/2020. 2. Staff # 3's sworn disclosure statement was dated 08/08/2020. Start date for Staff #3 was 08/07/2020. 3. Staff verified the dates the sworn disclosure statements were signed and the start date for both employees.
Plan of Correction:
In the future administration will ensure that all sworn disclosure statements are signed prior to the first day of employment.
