Comments:
An unannounced monitoring inspection was initiated and completed on 5/1/2024 from 2:45pm until 4:00pm. There were 17 school-age children and 3 staff present and 1 volunteer. Four children?s records. Children were observed playing outside, walking from school to the center, washing hands, having snack and doing homework.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violation (s) were documented on the violation notice issued to the program.
Based on record review, children's records did not have all the required information. Evidence: Child record #1 and #2 did not have emergency contact addresses listed.
Plan of Correction:
