A Subsidy Health and Safety Inspection (SHSI) was conducted on 4/2/2024 from 10:45am to 12:45pm. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.
Based on record review and interviews, it was determined the vendor did not ensure that staff who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training and staff development activities, to include the departments health and safety update course. 1.The record for staff #2 did not contain documentation of the 2023 health and safety update training. 2.The Staff #4 confirmed the annual training had not been completed.
