Comments:
An unannounced monitoring inspection was conducted on February 29, 2024 from approximately 9:10 am to 11:25 am. Six staff were present supervising twenty children ages infants through preschool age. Children were observed participating in circle time and playing freely inside and outside. Five staff and five child records were reviewed. Violations were found that are noted on the violation notice.
Based on review, facility did not ensure 1 of 5 child records has documentation of required information.
Evidence: The record for child # 4 (start date 10/16/2023) did not have a physical examination.
Plan of Correction:
Parent was contacted during inspection and she will bring a copy today.

Description:
Based on observation, facility did not ensure areas and equipment of the center are maintained in a clean, safe and operable condition.
Evidence: In the 2's classroom, there was a flat screen television located on a bookcase approximately 3' from the floor. The television was not secured and was within reach of children.

The tv will be removed from the classroom until it can be secured.

Description:
Repeat Violation Based on observation, facility did not have required items in the diapering areas.
Evidence: The following was found: 1. In the infant room, the diaper disposal system had a swivel top requiring a diaper or hand to open the lid. There was no covered receptacle for soiled linens. 2. In the toddler room, the step does not operate on the diaper disposal system requiring the lid to opened by a hand or diaper. 3. In the 2's classroom, the step is missing from the diaper disposal system requiring the lid to be opened by a hand or diaper.
Plan of Correction:
New trash cans with step will be purchased today and placed in diapering areas.
