Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 12/21/2021 and concluded on 12/21/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 0 children present, with 1 caregiver. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children's record and 2 caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the program.
Based on record review, the provider did not ensure two of two children's records contained documentation of the policy number of the child's medical insurance. Evidence: The record of child # 1 (start date: 11/30/21) and child #2 (start date: 7/5/21) did not contain documentation of each child's medical insurance policy number.
Plan of Correction:
I will call child #1 and child #2's mom to get the insurance policy number.

Description:
Based on record review, the provider did not ensure two of two children's records had documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before enrollment. Evidence: 1. The record of child #1 (start date: 11/30/21) had documentation of a immunization record that did not disclose if the child was adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations. 2. The record of child #2 (start date: 7/5/21) had documentation of immunizations dated 7/7/21.
Plan of Correction:
I will call child #1's mom to get a completed immunization record.
