Comments:
An unannounced monitoring inspection was conducted on November 6, 2023 from approximately 3:55 pm to 5:20 pm. Four staff were present with fifty-two school age children. Children were observed doing homework, eating a snack and participating in group activities. Five child and four staff records were reviewed. Violation was found that is noted on the violation notice.
Based on review, facility did not ensure 1 of 5 child records has documentation of required information.
Evidence: The record for child #1 (start date 9/5/2023) did not have a second emergency contact (other than parent), physical examination and immunization record.
Plan of Correction:
The Site Director will check all documentation prior to any student starting in programming to ensure all required documents are complete and accurate.
