Comments:
An unannounced monitoring inspection was conducted on April 29, 2024 from approximately 4:00 pm to 5:15 pm. Four staff were present supervising forty-eight school age children. Children were observed playing outside, working on homework and doing a variety of table activities. Four staff and five child records were reviewed. Violations are noted on the violation notice.
Based on review, facility did not ensure 1 of 5 child records has immunization and physical examination forms that are signed by a physician, designee or official of the health department.
Evidence: The record for Child #1 (start date 9/5/2023) has a physical and immunization record that is not signed by the physician, designee or health department official.
