Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
An announced initial inspection was initiated on September 2, 2020 and concluded on September 3, 2020. The area manager was contacted by telephone to initiate the inspection. The inspector emailed the owner a list of items required to complete the inspection. The inspector conducted a virtual inspection of the gymnasium and cafeteria. An open field will be used for outdoor play. The inspector reviewed background checks, staff training and staff qualifications for one staff submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined non-compliance(s) with licensing regulations, and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.
Based on review, facility did not ensure 4 of 5 agents noted on the business entity page of the initial application have completed background checks.
Evidence: The records for agent #1, agent #2 and agent #3 did not have a completed sworn statement or affirmation. The records for agent #1, #2, #3 and #4 did not have a completed search of the central registry check.
Plan of Correction:
Background checks and sworn statements have been completed and will be forwarded to LI.
