Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on March 10, 2021 and concluded on March 16, 2021. A self-reported incident was received by the department regarding allegations in the areas of care and protection of a child and following their own policies and procedures. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on review of written policies and procedures and staff interview, the center failed to ensure that the operational responsibilities of the license shall include, but not limited to, ensuring that the center's activities and services are maintained in compliance with the center's own policies and procedures that are required by these standards.
Evidence:
A center policy is in place to conduct a name to face check before feeding an infant any bottle. Staff is to ensure the name on the bottle is the name of the infant before feeding begins.
Staff #1 stated she did not confirm the name on the bottle before she began feeding an infant.
Staff #1 stated after feeding the bottle to the infant she observed the name on the bottle was not the name of the infant she was feeding.
Staff #1 stated she feed an infant the incorrect bottle of formula.

Plan of Correction: All staff in the infant room are retrained in infant feeding policy including the name to face check before feeding an infant any bottle.
Steps to ensure a child receives their correct bottle have been posted on the refrigerator and next to the bottle warmers.
Each child is assigned a tray in the refrigerator with their name to place their bottles and food.
Colored tape is used to label children's bottles and food. Each child is assigned a separate colored tape.

Description: Based on statements and interviews conducted, staff supervising children did not ensure their care, protection and guidance.
Evidence:
Staff #1 stated an infant was fed the incorrect bottle of formula.
Staff #1 stated the name to face feeding procedure was not conducted before feeding the infant.

Plan of Correction: All staff in the infant room are retrained in infant feeding policy including the name to face check before feeding an infant any bottle.
Steps to ensure a child receives their correct bottle have been posted on the refrigerator and next to the bottle warmers.
