Comments:
A monitoring inspection was conducted on March 30, 2022 concerning a self reported staff supervision incident. Interviews were conducted and electronic data was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on interview with management, the licensee did not ensure the center's compliance with their own policies and procedures. Evidence Staff #1 did not follow the center's own policy and procedure of threshold counts and name-to-face check to ensure that all children are accounted for the the group.
Plan of Correction:
We will continue to follow VA Licensing requirements/guidelines. We will also retrain staff on name-to-face procedure.

Description:
Based on interview with management, review of electronic information a child under 10 years of age was not always within actual sight and sound supervision of staff. Evidence Child #1 who is under the age of 10, was not always within actual sight and sound supervision of staff for approximately less than one minute.

We will continue to follow VA Licensing requirements/guidelines. We will also retrain staff on name-to-face procedure.
