Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the home.
A monitoring inspection was initiated on August 4, 2021 and concluded on August 4, 2021. The provider was contacted by video call and a virtual inspection was conducted. There were three children present, ranging in ages from eight months to 15 months with one provider. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, and medication. A total of two children?s records and two provider/household member records were reviewed.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review of four children?s records, observations and interview, the provider did not meet all the criteria on the health and safety checklist at the time of the inspection.
Evidence: 1. During the review of documented monthly emergency evacuation drills, the documented indicated an emergency evacuation drill had not been completed for March 2021. The providers confirmed an evacuation drill was not performed in March 2021. 3. The record for Child # 1 (first date of attendance 02/10/2021) contained immunization record dated 02/22/2021. The record for Child # 2 (first date of attendance 04/13/2021) contained a physical exam and immunization documentation dated 07/07/2021.
Plan of Correction:
1. I will make sure to complete an emergency evacuation drill each month. This was an oversight. 2. I will not allow children to begin attending until the required shot and physical exam records are submitted.

Description:
Based on an interview, the provider did not ensure a completed a sworn statement or affirmation prior was obtained for one adult household member prior to residing in the home and a search of central registry was not obtained within 30 days after residing in the home.
Evidence: The preliminary information gathered for the remote inspection indicated Household Member #1 was living in the home. The provider stated Household Member #1 moved into the home in March 2021 and a sworn statement or affirmation and a search of central registry has not been obtained for Household Member #1.
Plan of Correction:
I will have the household member complete or begin the required background checks process by the end of this week.

Description:
Based on an interview, the provider did not ensure the results of a fingerprint based background check was obtained within 30 days of one adult household member moving into the home.
Evidence: The preliminary information gathered for the remote inspection indicated Household Member #1 was living in the home. The provider stated Household Member #1 moved into the home in March 2021 and a fingerprint based background check has not been obtained for Household Member #1. Household Members are required to complete a national fingerprint background check through FieldPrint within 30 days of moving into the home.
Plan of Correction:
I will have the household member complete or begin the required background checks process by the end of this week.
