Comments:
Today an unannounced Monitoring Inspection took place between the hours of 1:30pm-2:55pm. There was a total of 42 children with 12 staff within the supervision guidelines. The children were observed taking a nap, coloring and drawing pictures and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Based on a review of staff records, each staff member did not submit documentation of a negative tuberculosis screening within the last 30 calendar days of the date of employment.
Evidence: 1. Staff #2 (doh: 11/6/2023) had a TB test on file that was dated 11/7/2023, which was after the date of hire. The TB test is required by day one of employment or within the last 30 calendar days prior to the date of employment.
Plan of Correction:
Moving forward we will make sure the TB screenings are completed in time.

Description:
Based on a review of staff records, 2 out of 3 staff records did not contain the required information.
1. Staff #1 (doh: 7/18/2023) did not have documentation of an emergency contact on file at the time of the inspection. 2. Staff #2 (doh: 11/6/2023) did not have documentation of an emergency contact and 1 of 2 required references of character and reputation on file at the time of the inspection.
Plan of Correction:
We will get the missing emergency contacts and reference today.
