Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A focused inspection was initiated on 12/22/2020 and concluded on 12/28/2020. The provider was contacted virtually to initiate the inspection. There were 2 children present and 2 caregivers. The inspector reviewed all items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 caregiver records to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Based on record review, the provider did not ensure that one of two children?s records contained documentation of a physical examination by or under the direction of a physician prior to the child?s attendance or within 30 days after the first day of attendance. Evidence: During the inspection the provider was not able to submit, to the licensing inspector, documentation of a physical examination for child #1 (start date: 11/10/2020). The document submitted to the licensing inspector did not have the child?s name, date of birth or any other identifiable information, physical examination results, and the date of the physical examination. The documentation only listed previous immunizations and upcoming health maintenance.

Description:
Based on record review, the provider did not ensure that one of two children?s record contained documentation of immunizations showing that the child has been adequately immunized according to the requirements of the Code of Virginia and applicable State Board of Health regulations. Evidence: The record of child #1 (start date: 11/10/2020) did not have the following documentation on the child?s immunization record; the child?s name, a statement that the child has been adequately immunized, signature or stamp and dated by a licensed physician, designee or local health department.
