06/21/2022 Arrived 1:30pm Departed 3:45pm An unannounced monitoring inspection was conducted today. There were 29 children present with three staff. The children were playing with having art, playing in the gym and outdoors. Thank you for your cooperation. Please contact me if I may be of service.
Based on a review of staff files, the facility failed to ensure that a fingerprint background check is completed for each employee prior to the first day of employment. Evidence: 1. There is no fingerprint background check on file for staff #3 who began work on 5-31-22.
