6/8/23 Arrived 9:20am Departed 11:15am An unannounced monitoring inspection was conducted today. There were 21 preschool children (ages 3-11years) present with four teachers. The children were arriving, washing their hands, playing with toys and playing outdoors. Five child records were reviewed. Thank you for your cooperation. Please contact me if I may be of service.
Based on a tour of the playground, the facility failed to ensure that all equipment is kept in safe and operable condition. Evidence: 1. On the climber, there is significant, flaking rust on the under side of the straight yellow slide and on the blue steps. 2. On the straight yellow slide there is a 1/4 inch gap between the slide and the deck. This presents an entanglement point.
Plan of Correction:
The program director reported that the climber will not be used by children until: the severity of the rust can be evaluated and repaired. the entanglement point between the slide and deck has been repaired.
