Comments:
A monitoring inspection was initiated on 11/16/2022 and concluded on 11/16/2022. There were a total of 3 children (12 points) observed with 2 caregivers present. The children were engaged in the following: toy play, and songs. The home, 3 child records, 6 caregiver/household member records, evacuation drills, emergency supplies were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of evacuation drills, the Provider has not conducted monthly fire evacuation drills. Evidence: The Provider did not conduct a fire drill for the months of September and October 2022.
Plan of Correction:
Fire drills will be conducted monthly and recorded on the form.

Description:
Based on a review of children records, the Provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child #3, first day of attendance 6/8/2022 does not have documentation of a physical examination.

The provider will ask the parents for a completed physical examination and file in the child's record.

Description:
Based on a review of children records, the provider did not obtain the parents written authorization for emergency medical care should an emergency occur. Evidence: Child # 's record1, date of first attendance 8/9/2022, was missing the written authorization from the parents for emergency medical care.
