Comments:
This inspection was conducted by licensing staff using an alternate remote protocol with an in-person component that included telephone contacts, documents review, interviews and a tour of the program with observations conducted at the center.

A complaint inspection was initiated on 11/1/2021and concluded on 1/5/2022. The in-person component was completed on 11/10/2022. There were seven children present, ranging in ages from two to three, with three staff supervising.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-80-B-2
Complaint related: No
Description: Based on interviews the center failed to inform the department's representative as soon as practicable, but not to exceed two business days, of an injury that required outside medical attention.
Evidence:
1. An injury to a child occurred on 10/28/2021. The center has not informed the department's representative of the injury that occurred that required medical attention.
2. When the guardian was contacted by staff # 4, she was told of the burn and a picture was text to the guardian.

Plan of Correction: The staff and or director will ensure that all injuries requiring outside medical treatment are reported to the department's representative within two business days.

Description: Based on interviews the staff failed to ensure the care and protection of all children in care.
Evidence:
1. On 10/28/2021 staff #1 was completing a craft with the children. Staff #1 gathered the supplies needed and plugged a hot glue gun into an outlet. The glue gun was then placed on top of a three foot cubby that is used to separate the areas. Staff #1 stated she was a few feet away and when she looked back at the glue gun child #1 (age: 2 years 11 months) had the glue gun in hand and had hot glue on her arm. The child was crying.
2. Staff # 1 & 2 removed the glue from the arm and applied first aid. Staff #4 called the guardian and reported the burn and sent a picture by text. Child #1 was taken to the doctor by the guardian.
Child #1 had a second degree burn.

Plan of Correction: Hot glue guns are not allowed, nothing that is child inappropriate is on the floor. Staff will continually monitor children whereabouts and actions.

Description: Based on interviews with staff children under 10 years of age were not always in actual sight supervision of staff.
Evidence:
1. On 10/28/21 staff #1 was gathering supplies to complete a craft. Staff # 1 stated that she plugged in a hot glue gun in the outlet next to the three foot cubby. She placed the glue gun on top of the cubby. She then began organizing the craft materials on the cubby. Staff #1 stated that she did not see the child pick up the glue gun. Staff #1's observation was fixed on the craft materials.
2. Staff # 2 was involved with an incident with two other children in the classroom.
3. Staff #3 was interviewed and stated that during the incident she was in the office getting some coloring sheets for the children and was not providing supervision to the children at that time.
4. All three staff stated they didn't see the child grab the glue gun when the incident occurred. They became aware when the child cried. Child #1(age: 2 years 11 months) had reached up and grabbed the hot glue gun, which resulted in hot glue getting on said child's left forearm. Child #1 had a second degree burn from the hot glue.

Plan of Correction: All staff will position themselves in order to have all children within sight and sound supervision at all times.
