Based on record reviews the center failed to ensure that staff shall complete orientation training prior to the staff working alone with children and no later than seven days of assuming job responsibilities. Evidence: Staff #1, 2, 3 did not have documentation of orientation training being completed prior to working alone with children.

Description:
Based on observation the center failed to ensure that hazardous shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. Located on the sink in the infant area was Clorox wipes, disinfectant spray not locked. 2. Located on a shelf next to the refrigerator in the infant area was glass cleaner, not locked.

Description:
Based on observation the center failed to ensure that children under 10 years of age were always within actual sight and sound supervision of staff. Evidence: In the pre-k area during nap time two children were placed behind shelves to sleep. The six staff in this area remained at a table eating lunch approximately 25 feet from the children. The staff could not see the two children during nap time.
Plan of Correction:
Ensure staff keeps sight and sound supervision of ALL children even during nap. [sic]
