Based on observation the center failed to have a copy of the special order provided by the Department to be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations. Evidence: The Notice of Intent, Special Order and violation notice were not posted in a prominent place at the public entrance. These are to be posted for one year.

Based on record reviews and interviews the center failed to ensure that lockdown procedures shall be practiced at least annually. The center did not have documentation that a lockdown drill was practiced in 2022.
