Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A subsidy health and safety initial inspection was initiated on 1/7/21 and concluded on 1/8/21. The person in charge was contacted by telephone to initiate the virtual inspection. The inspector provided the person in charge a list of items required to complete the inspection. The Inspector reviewed the administrator?s records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with subsidy regulations and violations were documented on the violation notice issued to the facility.
Based on observation and interview, the center failed to keep all hazardous substance in a locked place using a safe locked method to prevent access by children.
Evidence:
During the virtual tour of the facility, the hall restroom used by children was observed. The cabinet behind the toilet was opened by the administrator and it contained cleaners such as Clorox toilet bowl cleaner. The cabinet did not have a lock. The bathroom door remains unlocked when not in use.
Plan of Correction:
All cleaners were immediately removed and staff will be directed to not put hazardous substances in the restroom.

Evidence:
During the virtual walk through of the facility the administrator stated an emergency evacuation map had not been posted.

Based on interview, the center failed to post a 911 or local dial number for police, fire, and emergency medical services and the number for the regional poison control center in a visible and conspicuous place.
Evidence:
During the virtual tour of the facility the administrator stated a 911 or local dial number for police, fire, and emergency medical services and the number for the regional poison control center were not posted in a conspicuous place at the center.
Plan of Correction:
A 911 or local dial number for police, fire, and emergency medical services and the number for the regional poison control center will be posted in a conspicuous place at the center.
