Comments:
An unannounced monitoring inspection was conducted on, May 22, 2024. The inspector was on site from approximately 9:20 am until 10:45 am. There were four children present with two caregivers supervising for a value of 7 points. Children were observed participating in circle time. Children discussed the calendar and sang songs. Later children played with toys and traced their name on paper. The children also sang their own version of the ABC?s for the inspector. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, two caregiver records, and two household member records were reviewed. The inspectors interacted with the provider and children in care. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of four children?s records, the provider did not obtain the parent?s signed acknowledgement of the annual emergency contact information review.
Evidence: 1. The most recent signed review of the emergency contact information for child #2 (enrolled 09/07/2021) was dated 08/11/2022, exceeding the annual requirement. 2. The most recent signed review of the emergency contact information for child #4 (enrolled 09/07/2021) was dated 08/09/2022, exceeding the annual requirement. 3. The provider stated, parent had reviewed the information but had not signed the acknowledgement.
Plan of Correction:
Per the provider: I will have the parents sign the acknowledgement that the emergency information has been reviewed.

Description:
Based on review of four children?s records, the provider did not obtain documentation that a child had been adequately immunized before the child may attend the family day home.
Evidence: 1. The record of child #1, enrolled 11/20/2023, contained documentation of immunizations dated 12/05/2023, exceeding the first day of attendance. 2. The record of child #3, enrolled 11/13/2023, contained documentation of immunizations dated 12/05/2023, exceeding the first day of attendance.
Plan of Correction:
Per the provider: Going forward, I will obtain the immunization before the child attends.
