Comments:
An unannounced monitoring inspection was conducted on today, October 11th, 2022. There were 5 (16 points) children present with 2 caregiver's supervising. The home, children's records, the back yard, first aid kits, menu's, diaper creams, evacuation logs and emergency preparedness procedures were reviewed. The children were observed singing songs, playing in the backyard, hand washing and having lunch. Lunch consisted of chicken, noodles, mixed vegetables, pineapple and milk. Based on observation, the family day home failed to ensure that each building that houses a child day program that is licensed and that serves preschool-age children shall be equipped with at least one carbon monoxide detector on each floor where children are served.
Evidence: The provider did not have a carbon monoxide detector on the first floor where children are served.
Plan of Correction:
I will purchase a carbon monoxide detector for the first floor.

Description:
Based on observation, when topical skin products, such as diaper ointments are used, the product if provided by the parent, shall be labeled with the child's name.
Evidence: There were 4 containers of diaper cream that were not labeled with the child's name.

Description:
Based on a review of children's records, the family day home failed to obtain all required documents for each child enrolled.
Evidence: Child #3 and child #5's records do not contain signed written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. Child #1 and child #2's records do not contain documentation of an annual update of emergency contact information.
Plan of Correction:
I will complete annual review for children noted on violation notice. And I will monitor all records to keep them up to date.
