Comments:
An unannounced renewal inspection was conducted on April 3, 2023. There were 5 children (12 points) with 2 caregivers. The home, children's records, caregiver records, household member records, pet records, first aid kits, menu's and cribs were reviewed. Areas of non-compliance were cited on the violation notice. Based on a review of children's records, the provider failed to obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #3's record does not contain documentation of a physical exam by the 30th day of attendance. First Day of Attendance - 08/02/2021.
