6/14/23 Arrived 8:45am Departed 10:00am An unannounced renewal inspection was conducted today. There were 14 school-aged children present with four teachers. The children were arriving, having free choice time and preparing to go on a field trip. Five child records were reviewed. Staff records were reviewed remotely. Thank you for your cooperation. Please contact me if I may be of service.
Based on a tour of the van used to transport children, the facility failed to ensure that there is a document kept in vehicles containing local emergency contact information, potential shelters, hospitals, evacuation routes, etc.
Evidence: 1. There is no document in the van containing the required emergency information.
Plan of Correction:
The site director reported that the document will be completed and placed in each van used to transport children.
