Comments:
An investigation was begun in response to an allegation of lack of supervision, and physical plant concerns being received in our office on 10/12/23. Based on multiple staff interviews and an inspection of the playground area and fence, the preponderance of evidence does not support the allegation. The complaint is not valid.

Based on Head of School statement and a review of injury reports, the center failed to inform the department's representative of an injury that occurred at the center that required outside medical attention. Evidence: Child #1 sustained an injury on the playground. While climbing on the playground fence, the child was scratched on the leg. The child's parents were contacted and the child was taken to the doctor. The center was aware that the child did seek outside medical attention. No report was made to the department's representative within 2 business days.
