An unannounced monitoring inspection was conducted on 2/23/24 in response to a self-reported incident being received in our office.
Multiple staff interviews were conducted, injury reports were reviewed, and the playground was observed. See Violation Notice for non-compliance cited today.

Repeat Violation Systemic Deficiency Based on a review of injury reports, written reports did not contain all required information. Evidence: 10 injury reports were reviewed. All reports were dated after 2/7/24. 6 of 10 reports did not include the time/date or method of parent notification.
