Comments:
A code compliance inspection was conducted on April 22, 2024 from approximately 1:20 pm to 2:40 pm. There were thirty-four children present, ranging in ages from infants to preschool, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of four children?s records and four staff records were reviewed. Violation found is noted on the violation notice.
Based on review, facility did not ensure 1 of 4 staff have documentation within the past year of being certified by a practicing physician to be free from any disability that would prevent him from caring for children.
Evidence: The record for staff #2 had a most recent staff health form dated 3/7/2023 which is more than a year old.
Plan of Correction:
Staff will get it done this week.
