Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview, and a virtual tour of the program.
A monitoring inspection was initiated on 01/18/2022 and concluded on 01/20/2022. The provider or in-charge person was contacted by telephone to initiate the inspection. There was 1 child present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 1 child?s record and 1 provider record, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observation it was determined the first aid kit did not contain all of the required components. Evidence: There was no triangular bandage in the first aid kit.
Plan of Correction:
The provider will get a triangular bandage for the first aid kit.

Description:
Based on observation it was determined the provider did not ensure that all furnishings of the family day home were in a safe and operable condition. Evidence: The two shelves below the changing table were loose and cracking.

Provider will remove or repair the shelves.
