Comments:
An on site monitoring inspection was conducted on November 16, 2021. The provider was present and participated in the inspection. There were nine children present, ranging in ages from eight months to four years, with four caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records, four caregiver records and three household records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on interview, the provider did not ensure Medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. The provider stated household medications were being stored on top of the refrigerator, unlocked in a bag. 2. The refrigerator is on the second floor of the home where children do not have access to.
Plan of Correction:
Per the provider: I will remove the household medications and place them in a lock box.

Description:
Based on observation, the provider did not ensure crib sides were always up and the fastenings secured when a child is in the crib, except when the caregiver is giving the child immediate attention.
Evidence:
1. Two cribs were observed with the side folded down while each was occupied with a sleeping infant. 2. The caregiver #2 stated she had a hard time getting the infants in and out of the crib with the side up and the latch was difficult to secure.
Plan of Correction:
Per the provider: I will personally assist with the crib sides to ensure they are up and secured occupied by infants. Corrected while the inspector was on site: The crib sides secured in the upright position.
