The following was discussed with the licensee:
8 VAC20-800-620-A When providing care for a child with special needs, a written recommendation by the child's physician, psychologist, or other professional who has evaluated or treated the child should be obtained on how to care for and provide appropriate activities.
8 VAC 2-800 -620 F-G The provider and the parent of the child with special needs shall mutually determine a recommendation for the level of staffing necessary to care for and supervise the child based on the child's chronological and functional age and degree of disability.
Within 30 days of the child's enrollment, the provider shall provide the department's representative a written recommendation for the level of staffing necessary to care for and supervise the child
Comments:
An unannounced renewal inspection was conducted on 10/25/2021. Upon arrival there were 2 children in care. Both children were observed during nap. A total of 3 children's records and 4 caregiver/household records were reviewed. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a record review and interview the licensee did not have documentation that all children had a physical examination by a physician. Evidence: Child #1 and #2 did not have documentation of having a physical examination. The licensee confirmed she did not have a copy of the examinations.
Plan of Correction:
The licensee will contact the parents today and request physical examinations. The form will be printed out and given to parents for their convenience.
