A renewal inspection was conducted on 10/05/2022. A tour of the home was conducted, in addition to a record review. The Inspector reviewed 4 children?s records and 4 caregiver records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a record review the licensee did not ensure all caregivers/household members had documentation of the required out-of-state background checks. Evidence: Caregiver/Household members #1, #2, #3 and #4 did not documentation of having the required out-of-state background checks.
Plan of Correction:
The following corrective action was provided during the exit interview: All background checks have been done, however the licensee has misplaced the paperwork. Licensee in the process of trying obtain a copy of the fingerprints.

Description:
Based on a record review the licensee did not have documentation of having fingerprints for caregivers or household members. Evidence: Caregiver/Household members #1, #2, #3 and #4 did not documentation of having fingerprints.

Description:
Based on a record review the licensee did not ensure there was documentation of all children having had a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child #4, enrolled on 03/14/22, did not have documentation of having a physical examination.
Plan of Correction:
The following corrective action was provided during the exit interview: The child is disenrolling and will no longer be in the licensee's care.
Standard #:

Description:
Based on a record review the licensee did not ensure all requirements were met int he children's records.
Evidence: Child #2 (enrolled 04/04/2022) did not have the written acknowledgement that the child's parents were in receipt of information that is required to be given to parents. Such information includes: Operating information, fees, check in and out procedures, medication policies, ill child pick-up, communicable disease policy, immunization requirements, mandated reporting, custodial parents rights, general schedule, policies for food, discipline, emergency preparedness information and termination policies.
Plan of Correction:
The following corrective action was provided during the exit interview: The licensee will obtain the parent signature on the Information for Parents form.
