Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A renewal inspection was initiated on August 24, 2021 and concluded on August 25, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were five children present for a total of fifteen points, ranging in ages from five months to three years of age, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records, and two caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
If you have any questions or concerns please contact the Licensing Inspector at 540-430-9257.
Based on review of children's records, the provider failed to obtain all required information in a child's record.
Evidence: 1. Child #3's date of enrollment was 08/16/2021. The written authorization for emergency medical care should an emergency occur and the parent can not be located was not signed by the parent until 08/24/2021. 2. The provider verified the date of enrollment for Child #3 and the date the authorization for emergency medical care was signed.
Plan of Correction:
In the future the provider will review all paperwork to ensure all required paperwork is documented with signatures before or on the child's first day of attendance.
