Comments:
An unannounced monitoring inspection was conducted today from 1:15 pm-3:05 pm. There were 58 children (infants-5yrs.) directly supervised by 11 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping, being bottle fed on demand, waking up from nap and preparing for snack. Diapering and hand-washing procedures were also observed and discussed. There was an supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Based on record review, staff record was incomplete. Evidence: Staff A ( Date of Hire 2/13/2023) did not have reference #2 completed before employment.

Description:
Based upon observation, cribs were not provided for all children in care under 12 months of age. Evidence:
There There were 5 infants under the age of 12 months and 4 cribs available for nap time. The 5th infant ( 11 months) was asleep on a mat waiting for a crib.
Plan of Correction:
We will move another crib in the room until next month when the child will be 12 months.

Description:
Based on observation, 12 inches of space between occupied cots was not maintained.
Evidence: In the 2's classroom there were 3 cots where children were sleeping that were not spaced with at least 12 inches of space in between.
