Comments:
An unannounced monitoring inspection was conducted today from 9: 30 am- 10:50 am. There were 41 children (infants-5yrs.) directly supervised by 11 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating snack. Diapering and hand-washing procedures were also observed. There was an ample supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Based on observation, areas and equipment were found not maintained in safe condition.
Evidence: On the playground the log cabin, boat and yellow car were found to have exposed nails and splintered rough wood.

Evidence: Children in the toddler classroom were served grapes that were not cut before serving onto their plates at snack.
