Comments:
An announced initial Subsidy Health and Safety inspection was conducted today, October 29, 2021 and concluded on October 29, 2021 to determine the center's compliance with Subsidy Health and Safety vendor requirements. The inspection was initiated at 10:45am and concluded at approximately 1:35pm. There were no children present during this inspection. All subsidy requirements were reviewed and overall compliance determined. Administrative records and all areas of the center were inspected. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not ensure at least one staff has obtained within the last three years instruction in performing a daily health observation of children. Evidence: During the inspection of two staff records, it was discovered there were no staff that received training in daily health screenings.

Description:
Based on observation, the center did not ensure areas and equipment of the center, were maintained in a clean, safe, and operable condition. Evidence: 1. There were to two holes in the preschool floor which presented a tripping hazard. 2. The center's floor had several uneven areas which presented a tripping hazard.

We will let the center representative know so that a contractor can fix it by 11/19/2021.

Description:
Based on observation and an interview, the center did not ensure to provide cots, rest mats or beds for children during designated rest periods. Evidence: The center director stated that the center did not have any cots or rest mats at the time of the inspection.

Description:
Based on record review, the center did not ensure that the emergency preparedness plan contained all of the required procedural components. Evidence: The center's emergency preparedness plan did not have the following: 1. Method to ensure essential documents, including emergency contact information, attendance records, medications, and supplies are taken to the assembly point and relocation site. 2. Procedures to address reuniting children with a parent or an authorized person designated by the parent to pick up the child. 3. Staff training requirements, drill frequency, and plan review and update. 4. Continuity of operations procedures to ensure that essential functions are maintained during an emergency.
