Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 4/21/21 and concluded on 4/23/21. The provider was contacted by telephone to initiate the inspection. There were 6 children present with the provider. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and the provider?s record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and interview, the caregiver failed to provide the parents all the required information.
Evidence:
1. The written agreement from parents that includes information regarding the provider's liability insurance for child1 and child 2 was not provided. 2. The provider stated she thought she only needed that if she had liability insurance.
Plan of Correction:
The document regarding the family day home's liability insurance will be given to the parents of child 1 and child 2 for signature. In the future all children that enroll will be provided with the insurance liability form to sign.
