Technical Assistance:
Consultation was given on MAT training, auditing medication forms and the medication box monthly to ensure all authorizations/medicatios are current and allergy plans can be completed as listed.

A cot or mat must be provided for all children and the child should be placed on the carpet area during rest time.

Comments:
An allegation was received in the Fairfax Licensing Office concerning care of children. A joint investigation was conducted with the local Child Care office. Reviewed medication records, staff interviews and records. The allegation for care of children is not valid. However other violations during the investigations were noted. Please be aware that the plan of correction must be provided within 5 days of receipt of the inspection notes if you want it displayed on the website. Description: Based on observation, a cot or rest mat was not provided for a child during rest time.
Evidence:
Child #1 was directly on the floor without a mat or cot. The center staff stated the child refuses to sleep on anything.

Description: Based on medication review, the center failed to follow medication procedures.
Evidence:
1. One medication was outdated for Child #2-expired 11/6/2023.
2. Long term authorization expired for medication on 9/22/2023 for Child #2's emergency medication.
3. Action plan was older than one year (9/22/2022)

Plan of Correction: Child was provided with a giant sleeping mat at that time that was given by the parent. We will also be providing a cot per advice on 11/16/2023.
