Comments:
Today an unannounced Renewal Inspection took place between the hours of 1:40pm-3:00pm. There was a total of 41 children with 11 staff within the supervision guidelines. The children were observed playing games outside, in specialty camps, doing art projects and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, medication and emergency procedures were observed during this inspection. *Repeat Violation* *Systemic Deficiency* Based on a review of staff records, the facility did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years for 1 out of 4 staff records reviewed. Evidence: 1. Staff #4 (doh: 7/17/2023) did not have the results of state #1's (in which the employee resided in the preceding five years) child abuse and neglect registry search on file during the inspection. 2. 1.
