A complaint investigation was initiated on 02/08/2022 and concluded on 08/23/2022 in response to a complaint that was received by the department on 02/08/2022 regarding supervision, injury reporting, and administration. The inspector conducted an unannounced inspection on-site on 02/16/2022 from 11:45am to approximately 12:30pm. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. Additional interviews were conducted off-site and additional information was submitted to the inspector electronically. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint was determined to be not valid. There are no violations resulting from this inspection.
