Comments:
An unannounced monitoring inspection was conducted on, June 16, 2022 to determine compliance with the family day home licensing standards. The inspector arrived at 9:17 AM and concluded the inspection at 11:02 AM. There were a total of 11 children being supervised by the provider and 3 assistants. Upon the inspectors arrival the children were playing inside with various play materials. The children were later observed by the licensing inspector in another designated area in the home, interacting with peers singing and dancing. The outdoor play area, inside play areas, restroom, and kitchen area were inspected. Five children's records, three staff records, fire drill and shelter in place drill logs, first aid kit, liability insurance, staff training and all required postings were reviewed. The provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and are documented on the violation notice.

Based on a review of documentation, the TB results for one of three staff were completed within 30 days prior to hire. Evidence: The TB on file for Staff #2 (DOH: 5/2/2022) was completed 11/5/2021.

Description:
Based on observation, the provider did not ensure that all of the areas inside the family day home were maintained in a safe condition. Evidence: 1. Three outlet cover plates were missing from the electrical outlets in the dining room.
Plan of Correction:
The provider stated that painting will be done June 17th and the outlet plate covers will be put back at that time.

Description:
Based on observation provider did not ensure potentially poisonous substances are stored in areas inaccessible to children. Evidence 1. Bug spray was left on the counter top near the stove. 2. Bug repellent was left outside on the deck accessible to children.
Plan of Correction:
The provider put the bug spray in a locked cabinet under the sink.
